How do I prepare for and attend my First intake appointment?

1. Fill out and submit an Intake Form at least 7 days (1 week) prior to your appointment.

If the appropriate form is not filled out and submitted before your appointment we may have to reschedule your appointment.

2. Your appointment will be at 418 Eglinton Avenue WEST, 2nd Floor (within the Magenta Health clinic), Toronto.

IMPORTANT NOTE: To ensure you are directed to the correct building, please be sure to include "WEST" into your map app, otherwise the directions may default to "east". Please double check that you are being directed to the correct location. The Magenta Health clinic is located near Avenue Road - please see the map at the bottom of this page.

3. Plan for your appointment to take up to 90 minutes.

4. Please arrive 15 minutes early for your appointment to allow time for screening and any other appointment prep required by the physician. Please keep in mind there is currently an increase in traffic congestion due to construction. Kindly take this into account when planning your travel to the clinic.

5. If you arrive late for your appointment, there is a possibility that you will not be seen, or that you will be asked to wait until the next break in the schedule. This is done in order to help your doctor stay on schedule.

6. Cancellation: Please show consideration to your fellow patients by cancelling or rescheduling appointments as early as possible. A fee will be payable in the event of a missed appointment, or appointments cancelled less than 7 days in advance. 7. Self-Screening for In-Person Appointments:

Within the past 5 days have you or any individuals in your group had any of the following symptoms?

  • Fever and/or chills, cough, shortness of breath, sore throat, headache, decrease in sense of smell or taste, runny nose, or nasal congestion?

If you do not have any of the symptoms listed: You have screened NEGATIVE. You may continue with your in-person appointment, and nothing further is needed from you.

If you do have any of the symptoms listed: You have screened POSITIVE. You may continue with your in-person appointment but kindly:

  • bring a mask to wear within the clinic at all times, including when you are in the exam room

  • complete a Rapid Antigen COVID-19 test prior to your appointment if possible (find a participating pharmacy/grocery location here). Having the results from a COVID-19 test is helpful to ensure appropriate precautions are taken to keep the community, staff, and physicians safe.


How do i schedule my intake appointments?

Referrals are required for all patients.

First, you need to be referred by another doctor. If we're able to accept the referral, we will let that referring doctor know.

We will then add you to a waiting list that we work through in order, based on when you were referred (your referral date).

Please refer to this page to see roughly how long you need to wait and/or where in the waiting list we are.

When you get to the top of the waiting list, we will call and text you. If we don’t reach you we will try to leave you a message (voicemail and/or text).

Once you receive our message, you can schedule online, or you can call our booking service between the hours of 11AM and 1PM, Monday to Thursday, at the extension supplied in our message.

Please note that the booking agents will not be able to help you with medical or administrative questions outside of questions related to booking an appointment. Additionally, all available appointments are shown online. The booking agents do not have the ability to make exceptions.

We do not have a cancellation list. If a convenient time is not currently available, please check back on the online booking page at a later time, as cancelled and rescheduled appointments will be made available to book.

Hormone Therapy referrals will require 2+ intakes.
Surgery Readiness referrals will require 1 intake.

How do I reschedule or cancel My Intake appointment?

To reschedule:

If you booked online, your confirmation email will include a link to easily reschedule. Otherwise, you can call our office between 11AM and 1PM, Monday to Thursday, at 647.812.2089 ext. 225.

To cancel:

If you booked online, your confirmation email will include a link to easily cancel. 

Otherwise, you can use the form below to send us a message, or call our office at 647.812.2089 anytime, to leave a voicemail.

If you cancel your appointment, you will have 6 weeks to schedule another appointment. After that point you will need a new referral.

Frequently asked questions:

  • Dr. Wong is a family physician with a focus in gender-affirming hormone therapy and transition related surgery assessments for gender diverse people.

    They earned their medical degree at the University of British Columbia and completed their Family Medicine training at the University of Calgary.

    They also trained at the Sherbourne Health Centre and Rainbow Health Ontario.

  • Dr. Wong’s practice includes adult patients (18+) who identify as trans, genderqueer, gender non-conforming, transexual, or non-binary, who are planning to start gender-affirming hormone therapy or apply for transition related surgeries

  • At this time Dr. Wong is unable to accept referrals for:

    • Children under the age of 18 (please send these referrals to the Transgender Youth Clinic at SickKids Hospital)

    • Primary care

  • Yes, Ministry of Health regulations require all patients to be referred to Dr. Wong from another physician.

    Please have a referring physician (e.g. your family doctor) fax a referral to 416.572.7103.

  • At the moment, patients must be covered by OHIP and not other provincial or third-party health plans.

  • Medically necessary procedures and services are generally covered by OHIP.

    Forms and letters may require private payment.

  • Due to Ministry of Health requirements, hormone therapy consultations are in person, and follow up visits are typically conducted virtually at regular intervals for 1 year until treatment is optimized.

    Surgery planning consultations consist of one appointment in person. Follow ups typically are not needed as the purpose of this consultation is to provide a letter to your surgeon and complete the form required for prior approval from the Ministry of Health.
