How do I prepare for and attend my intake appointment?

Virtual Intakes

1. Fill out and submit an Intake Form at least 7 days (1 week) before your appointment. This form is only for patients who have already booked an appointment. If this form is not filled out and submitted before your appointment we may have to reschedule your appointment.

2. Have a piece of government issued I.D. with you. This will be used during your appointment to validate your identity.

3. Privacy: Arrange to have your virtual appointment in a private and quiet room where you will not be interrupted by other people or disturbed by noise.

4. Plan for your appointment to take up to 2 hours.  

5. Kindly note all virtual appointments will be VIDEO only (i.e. it will not be an option to attend virtual appointments via phone). We appreciate your understanding and cooperation in this matter.

5. Cancellation: Please show consideration to your fellow patients by cancelling or rescheduling appointments as early as possible. A fee will be payable in the event of a missed appointment, or appointments cancelled less than 48 hours in advance. Information about how to reschedule or cancel can be found on this page on this page under the section titled “How do I reschedule or cancel My Intake appointment?”.

Access Dr. Monsour’s Zoom virtual room at least 5 minutes before your scheduled appointment time.
Dr. Monsour will bring you into the virtual room when done with the patient before you.

Please note that Dr. Monsour may be running up to 1 hour behind due to complexity of care.
Kindly remain in the waiting room, and Dr. Monsour will let you in as soon as he can. Rest assured your appointment will still happen, we do not cancel any appointments without notifying you.

Make sure to use your name - otherwise you will not be admitted to the room.


How do I prepare for and attend my intake appointment?

IN-PERSON INTAKES

1. Fill out and submit an Intake Form at least 7 days (1 week) before your appointment. This form is only for patients who have already booked an appointment. If this form is not filled out and submitted before your appointment we may have to reschedule your appointment.

2. Appointment Location: Please ensure that you check your appointment confirmation email for the address of where you will be seen. Dr. Monsour sees patients at more than one location.

3. Plan for your appointment to take up to 2 hours.

4. Cancellation: Please show consideration to your fellow patients by cancelling or rescheduling appointments as early as possible. A fee will be payable in the event of a missed appointment, or appointments cancelled less than 48 hours in advance. Information about how to reschedule or cancel can be found on this page under the section titled “How do I reschedule or cancel My Intake appointment?”.

5. Arriving: Please arrive 15 minutes early for your appointment to allow time for screening and any other appointment prep required by the physician.

Please keep in mind there is currently an increase in traffic congestion due to construction. Kindly take this into account when planning your travel to the clinic.

6. Late Arrivals: To help your doctor stay on schedule, if you arrive late for your appointment, there is a possibility that you will not be seen, or that you will be asked to wait until the next break in the schedule.

7. Self-Screening for In-Person Appointments:

Within the past 5 days have you or any individuals in your group had any of the following symptoms?

  • Fever and/or chills, cough, shortness of breath, sore throat, headache, decrease in sense of smell or taste, runny nose, or nasal congestion?


If you do not have any of the symptoms listed:
You have screened NEGATIVE. You may continue with your in-person appointment, and nothing further is needed from you.


If you do have any of the symptoms listed:
You have screened POSITIVE. You may continue with your in-person appointment but kindly:

  • bring a mask to wear within the clinic at all times, including when you are in the exam room

  • complete a Rapid Antigen COVID-19 test prior to your appointment if possible (find a participating pharmacy/grocery location here). Having the results from a COVID-19 test is helpful to ensure appropriate precautions are taken to keep the community, staff, and physicians safe.


HOW DO I SCHEDULE MY FIRST APPOINTMENT
(I.E. INTAKE APPOINTMENT)?

First, you need to be referred by another doctor. If we're able to accept the referral, we will let that referring doctor know.

We will then add you to a waiting list that we work through in order, based on when you were referred (your referral date).

Please refer to this page to see roughly how long you need to wait and/or where in the waiting list we are.

When you get to the top of the waiting list, we will call and text you. If we don’t reach you we will try to leave you a message (voicemail and/or text).

Once you receive our message, you can schedule online, or you can call our booking service between the hours of 11AM and 1PM, Monday to Thursday, at the extension supplied in our message.

Please note that the booking agents will not be able to help you with medical or administrative questions outside of questions related to booking an appointment. Additionally, all available appointments are shown online. The booking agents do not have the ability to make exceptions.

Need to cancel or reschedule your intake appointment?

Please show consideration to your fellow patients by cancelling or rescheduling appointments as early as possible. Kindly note that a fee will be payable in the event of a missed appointment, or appointments cancelled less than 48 hours in advance.

  • Rescheduling your intake appointment:

    If you booked your appointment online, your confirmation email will include a link to easily reschedule.

    Otherwise, you can call our office between 11am and 1pm, Monday to Thursday at 647.483.0690 ext. 225.

  • Cancelling your intake appointment:

    If you booked online, your confirmation email will include a link to easily cancel. Otherwise, you can use the form below to send us a message, or call our office at 647.483.0690 anytime, to leave a voicemail.

    If you cancel your appointment, you will have 6 weeks to schedule another appointment. After that point you will need a new referral.

    Rebooking Cancelled Appointments:

    You can schedule another appointment here or by calling our office between 11AM and 1PM, Monday to Thursday, at 647.483.0690 ext. 225.

  • Dr. Monsour is a neurologist specializing in Headache Medicine. Dr. Monsour has completed both a residency in Adult Neurology and a Fellowship in Headache Medicine at the University of Toronto.

  • Dr. Monsour’s practice interests include:

    • Management of headache ages 12 and up

    • Managing complex or refractory migraine

    • Trigeminal Autonomic Cephalalgias such as Cluster headache, Hemicania continua, Paroxysmal hemicrania, and Short-lasting Unilateral Neuralgiform Headache Attacks

    • Atypical headache disorders such as secondary headaches and headaches associated with high/low CSF pressure.

    • Procedural management of headache disorders including botulinum toxin injections and cranial nerve blocks.

  • Yes, Ministry of Health regulations require all patients to be referred to Dr. Monsour from another physician.

    Please have a referring physician (e.g. your family doctor) fax a referral to 647.480.5031.

  • At the moment, patients must be covered by OHIP and not other provincial or third-party health plans.

  • Medically necessary procedures and services are generally covered by OHIP.

    No direct patient payment is required.

  • We do not have a cancellation list. If a convenient time is not currently available, please check back on the online booking page at a later time, as cancelled and rescheduled appointments will be made available to book.
