 Otherwise, Ministry of Health regulations require a new referral. Please have a referring physician (e.g. your family doctor) fax a referral to 647.689.3346. If we're able to accept the referral, we'll contact you directly to schedule an appointment within approximately 5 business days.

How do I reschedule or cancel my appointment?

If you consented to receiving an email confirmation, you should have been sent an email confirmation with a link to cancel or reschedule. For most individuals, your credentials to cancel or reschedule your appointment:

Username: your email address
Temporary password: your OHIP number.

If you already happen to have an account with Veribook, our online scheduling system, you will need to use your existing account password instead.

You can also sign into your account directly here, if you lost the email confirmation.

Please show consideration to your fellow patients by cancelling appointments as early as possible if you no longer require an appointment or can no longer attend. Kindly note that missed appointments, or those cancelled without sufficient notice will be subject to a fee, payable before another appointment may be scheduled. Sufficient notice is: 48 hours for virtual appointments and 72 hours for in-person appointments.

It is also an option to call our booking service between 11 AM and 1 PM, Monday to Thursday, at 647.478.2580 ext. 225 to schedule appointments. Please note that the booking agents will not be able to help you with medical or administrative questions outside of questions related to booking an appointment.


HOW DO I PREPARE FOR and attend MY Virtual Follow-Up APPOINTMENT?

1) Access Dr. Plant’s Zoom virtual waiting room at least 5 minutes before your scheduled appointment time. Dr. Plant will bring you into the virtual room when he is done with the patient before you. Unfortunately, to help your doctor stay on schedule, if you arrive late for your appointment, there is a possibility that you will not be seen, or that you will be asked to wait until the next break in the schedule. Please plan to stay for up to 1 hour.

Make sure to use your name - otherwise you will not be admitted to the room.

2) Please show consideration to your fellow patients by cancelling or rescheduling appointments as early as possible. Kindly note that a fee will be payable in the event of a missed appointment, or appointments cancelled less than 48 hours in advance. Information about how to reschedule or cancel can be found on this page under “How do I reschedule or cancel a scheduled appointment?”.

3) Please note that your appointment may not start right on time. Please be patient with us and know that your flexibility is vital in keeping our community safe. 

4) Arrange to have your virtual appointment in a private and quiet room where you will not be interrupted by other people or disturbed by noise.

5) When connecting to your virtual appointment please use a private computer, phone, or tablet, with a secure internet connection and camera.  


HOW DO I PREPARE FOR and attend MY In-Person follow-up or procedure APPOINTMENT?

Before your Appointment (for all appointments)

1) Please shower as you normally would, wash the area with soap and water, and do not apply moisturizer or other products such as make-up to the area.

2) Please wear comfortable clothing that will allow you to easily expose the part of your body that Dr. Plant will be working on.

3) Change your clothes before coming to the clinic  and when arriving at home from the clinic.

4) Please show consideration to your fellow patients by cancelling or rescheduling appointments as early as possible. Kindly note that a fee will be payable in the event of a missed appointment, or appointments cancelled less than 72 hours in advance. Before your appointment (for procedures only)

5) Before your procedure you may eat and drink as normal.

6) Before your procedure please take your medications as prescribed, with the exception of the following.

  • Please do not take Aspirin (ASA), Plavix or non-steroidal anti inflammatory medications (NSAID’s) including:  Chondroitin, Glucosamine, Ephedra, Echinachea, Ginko, Ginseng, Kava, Garlic, St. John’s Wort, Ginger and fish oil for 7-14 days prior to your procedure. 

  • If you take Coumadin or Pradax, please stop 5 days prior to surgery and please check this with your family doctor to make sure this is appropriate.

The day of your appointment:

7) Your appointment will be on the 2nd Floor, 418 Eglinton Avenue West, Toronto, ON.

8) Do not arrive more than 5 min before your appointment time. If you arrive more than 5 minutes before your appointment you will need to wait elsewhere until your appointment time.

9) Wear a mask if you have one.  

10) If you arrive late for your appointment, there is a possibility that you will not be seen, or that you will be asked to wait until the next break in the schedule. This is done in order to help your doctor stay on schedule.

11) If your appointment is a follow-up or a 10 minute procedure please plan to stay for up to 1 hour.

12) If your appointment is a 30 or 60 minute procedure please plan to stay for up to 1.5 hours.

13) Cosmetic services are not covered by OHIP. We only accept credit card and cash. For cash payments we will not be able to make change. 

14) Do not come into the clinic if you can answer “Yes” to any of the below questions. 

  • Have you experienced any of the following symptoms in the last 14 days? Fever or chills, new or worsening cough, shortness of breath, muscle and/or joint aches, fatigue, sore throat, headache, runny nose, nausea, vomiting, diarrhea, abdominal pain, loss of sense of smell or taste, loss of appetite or weight, sneezing, nasal congestion, conjunctivitis (pink eye)?

  • Have you been in close contact with anyone who was experiencing any of previously mentioned symptoms in the last 14 days? 

  • Have you or anyone you are in close contact with, been diagnosed with COVID-19 in the last 14 days?

  • Have you or anyone you are in close contact with, traveled outside of Ontario in the last 14 days?

15) Know that we are following best practices to ensure social distancing and clinic safety.

Unfortunately, Ministry of Health regulations do not allow Dr. Plant to receive any compensation for providing medical advice remotely (e.g. by phone or in writing).

Nevertheless, for your convenience, if you have been seen by Dr. Plant within the past two (2) weeks and your question is very straightforward, please use the following form and Dr. Plant to see whether he can effective supply an answer without your coming in. Please do recognize that many questions will require an in-person assessment, and you may be asked to book a follow-up appointment instead.

For complex questions or if you have not seen Dr.  Plant within the past two (2) weeks, please follow the instructions above to schedule a follow-up appointment with Dr. Plant instead.
